Tantrum (2015)

A man perhaps affected by the death of someone close, maybe his grandma, begins wandering in the snow before turning his attention to himself. He cuts himself up, shoots himself in the head, picks at a boil on his face, cuts off his dick with scissors, hammers at a dead rat while rubbing one out, murders a jogger, rips his ass apart with a crucifix, before finally going after a woman dressed like his grandma. Or maybe it was her he was missing all the while. Who cares. Just nice practical effects.
